  17. called "Original  Agreement", pursuant to which _________

  21.       WHEREAS, Lessor and Lessee desire at this time to
  22. further extend and amend said original agreement as

  25. 2.    NOW, THEREFORE, it is mutually agreed that the
  26. "Original Agreement" shall be further extended and
  27. amended as follows:

  29.      (a)  The payments under this agreement shall continue
  30.           in effect for the period of _____________months

  32.           pay a monthly rental of _____________ plus
  33.           applicable sales tax for each and every calendar
